Lifestyle Medicine is an Evidence-based Practice
Dear Physician,
 
Thank you for referring your patient to us for improvements in their overall lifestyle, weight loss, diabetes management and cardiovascular improvement. We also support changes in dietary habits towards whole food plant-based nutrition, or support those patients that have already started down this path. We practice evidence-based medicine with an emphasis on whole food plant-based nutrition. We aim to maximize the patient's health potential through behavior modification while managing medical issues. We recognize that for chronic conditions, Behavior is the most powerful determinant but also the most difficult to modify. It is our hope that working with our multi-disciplinary team, we will have a significant impact on your patient's health.  
 
The five key areas of lifestyle medicine include whole food plant-based nutrition, physical activity, sleep improvement, emotional well-being and a reduction or elimination of substance use. This is all completed while managing medical issues utilizing evidence-based practices.
